Abstract

Type 2 diabetes mellitus (T2DM) and hypertension are chronic diseases with high prevalence that represent a significant health burden both globally and nationally. This condition calls for effective early detection strategies, one of which is screening at pharmacies. This study aims to analyze pharmacists' and patients' perceptions of T2DM and hypertension screening services at pharmacies in Madiun City using the COM-B framework (Capability, Opportunity, Motivation–Behaviour), employing a qualitative approach through semi-structured interviews with 7 pharmacists (purposive sampling) and a quantitative descriptive approach through dichotomous questionnaires administered to 50 patients (consecutive sampling). The results showed that pharmacists possessed adequate basic technical skills and strong information, education, and communication (IEC) competencies, but required further training and faced regulatory ambiguity as the greatest opportunity-related barrier, despite very high public acceptance; in terms of motivation, pharmacists demonstrated genuine concern and strong professional confidence despite being constrained by regulatory ambivalence. Meanwhile, patients' perceptions were categorized as good across all COM-B components, with Capability scores of 92% and 88%, Opportunity scores of 92% and 84%, and Motivation scores of 96% and 96%, indicating very high public interest in and acceptance of screening services at pharmacies.
